This 27th Nephrology and Primary Care Conference presents a renewed program and a dynamic format, aiming to continue bringing closer and strengthening collaboration between Nephrology and Family Medicine professionals in our territory. This alliance is key to improving the care of people with chronic kidney disease and hypertension, ensuring a coordinated, resolutive, and patient-centered response.

This year’s program addresses current topics of high clinical impact, with a special emphasis on the contributions of the MRC Working Group in the territory. In a context of sustained increase in the prevalence of MRC —driven by aging, diabetes, and hypertension— effective coordination that integrates health plans for chronic pathologies while preserving the efficiency of actions becomes essential.

We want to highlight that the Working Group has been awarded in the “II Program for recognition and dissemination of good practices/health interventions in the field of chronic diseases in the Catalan health system,” promoted by the Research Center in Economics and Health (CRES) of Pompeu Fabra University, for the project 25 years of integrated care for chronic kidney disease: a consolidated model of collaboration between Nephrology and Primary Care.” This recognition encourages us to continue advancing in the care of renal health and, above all, to keep demonstrating the effectiveness and efficiency of this care model.

We thank the support of the hospital management and the Primary Care teams, and we hope that this conference will be a useful space for dialogue and learning to consolidate tangible improvements for our patients.
