Nursing

A cross-departmental unit at all stages of the care process, alongside each patient and their caregivers.

At Fundació Puigvert, the nursing team is present at all stages of the care process and supports each person, their family, or caregivers in health promotion and prevention, diagnosis, treatment, and throughout their health and illness experience (including all possible outcomes, chronic diseases…). The team consists of nursing professionals, advanced practice nurses, nursing auxiliary care technicians, and specialists in laboratory, pathology, and radiology.

What makes our nursing unique

Our Nursing Management coordinates all functional areas of hospitalization, surgical area, outpatient nephrology, urology and andrology, reproductive medicine, emergencies, diagnostics (laboratories and radiology), ensuring actions aligned with the strategic plan and institutional values of the Fundació.

The shared goal is to ensure maximum quality, safety, and expertise in both assistance and in teaching and research.

Some elements that make nursing at Fundació Puigvert unique are:

Advanced Practice Nursing (APN) in renal damage prevention, advanced chronic kidney disease, peritoneal dialysis, home hemodialysis, transplantation, stomatherapy and complex care, urological endoscopy, functional and female urology, human reproduction, admission preparation and surgery, anesthesiology, infection control and case management, with roles in clinical leadership, teaching and research.

Therapeutic education and patient empowerment programs (chronic kidney disease, hemodialysis and home dialysis, renal damage prevention, ostomies, sexual and reproductive health, etc.), to promote self-care and shared decision-making.

Constant multidisciplinary work with nephrology, urology, andrology, reproductive medicine, radiology, laboratories and clinical psychology, to provide comprehensive and coordinated care.

Major areas of nursing

Outpatient Nephrology

In outpatient nephrology, advanced practice nursing supports individuals with chronic kidney disease, difficult-to-control hypertension, and other renal pathologies, both in early and advanced stages. The goal is to delay disease progression, prevent complications, prepare access for substitute treatments, and promote self-care with clear and personalized information.

Nurses in the Chronic Kidney Disease and Renal Damage Prevention, Transplantation clinics, as well as in the Nephrology Day Hospital, perform clinical assessments, diagnostic tests, monitoring of vascular access and the evolution of health and disease processes, in close coordination with nephrologists. In the hemodialysis and peritoneal dialysis units, the nursing team combines a high technical level with a very intense educational and supportive role, including home dialysis programs and predictive tele-assistance.
The Patient Classroom, Mentoring, and Expert Patient projects in Chronic Kidney Disease promote patient empowerment, shared decision-making, and treatment adherence.

Outpatient Urology

In outpatient urology, the nursing team carries out care follow-up, treatments, therapeutic education, and diagnostic tests. The areas of action are diverse: In functional and female urology, we have nurses with dual qualifications in physiotherapy, who lead urodynamic studies, flowmeters, and perineal re-education programs, promoting lifestyle changes and conservative treatments for urinary incontinence and other dysfunctions.

The urological and stomatherapy nursing consultation supports individuals who need an ostomy from the moment of diagnosis, in preparation for surgery and in returning to daily life, ensuring safe care of the stoma and peristomal skin.

The team offers comprehensive care that combines personalized information, therapeutic education, and emotional support to promote shared decision-making and the recovery of patient autonomy. In the field of outpatient urology, and especially in urological endoscopy techniques, advanced practice nursing (APN) nurses play a key role in training in the use of devices, support before and after the procedure, and the gradual adaptation of the person to their new health situation.

Outpatient Andrology

Fundació Puigvert has the only dedicated andrology nursing consultation in Catalonia, led by an advanced practice nurse specialized in male sexual and reproductive health. This consultation offers comprehensive and personalized care in erectile dysfunction, ejaculatory disorders, genital pain, hypogonadism, and semen donation processes, always working in coordination with the andrology medical team.

Outpatient assisted reproduction

In outpatient assisted reproduction, advanced practice nurses and nursing care technicians are part of a highly specialized team that supports individuals and couples with difficulties conceiving, single women, and individuals who wish to preserve fertility.

The nursing team coordinates visits, tests, medication schedules, preparation for inseminations and in vitro fertilizations, and provides ongoing information throughout the process.

Pre-admission, admission, surgery, and Emergencies

At the time of pre-admission, the nurses of the Functional Unit of Admissions and Discharges become the point of reference for the person and their family or caregivers, planning and preparing for admission or surgery in coordination with admissions and surgical planning. Their role is to inform, resolve doubts, and ensure that the patient arrives in the best possible condition for the procedure, both physically and emotionally.

In the event that the person requires immediate attention, the Emergency Service, operational 24 hours a day, 365 days a year, guarantees a rapid assessment and assistance for any emergency related to the specialties of Fundació Puigvert, working in coordination with the rest of the medical and nursing services to provide a comprehensive and continuous response.

The Pre-surgical and Major Ambulatory Surgery Unit, along with the surgical block and anesthesia nursing team, ensures safety and comfort before, during, and after the intervention, with special expertise in urological robotic surgery and minimally invasive procedures. In the hospitalization units, nurses and TCAI provide 24-hour care, clinical monitoring, pain control, education on healthy lifestyle habits, and discharge planning, creating a bond of trust with the patient and family.

In Emergencies of urology, nephrology, and andrology, the nursing team plays a key role in the rapid assessment of symptoms, the application of initial treatments, and the continuous monitoring of vital signs, ensuring comprehensive and humanized care in situations of maximum vulnerability. Their experience in managing catheters, probes, and specific devices is essential to ensure agile and safe clinical decisions, in coordination with the rest of the multidisciplinary team.

Continuity of care and safety

The case manager nurse ensures continuity of care between the different services of the Foundation and with the rest of the healthcare system, accompanying the patient and family, or caregivers during complex transition moments (admission, discharge, referrals, treatment changes). They also act as a clinical coordination hub, ensuring continuity of care and adding value in the Catalan, National, and European Expertise Networks in which we are accredited. Their work facilitates a more organized and safe experience, reducing avoidable readmissions and improving follow-up at home.

The infection control nurse is part of the Nosocomial Infection Control Team and is tasked with preventing, monitoring, and controlling infections related to care, both in patients and professionals. They develop protocols, provide specific training in hand hygiene and safety measures, and participate in quality and patient safety policies, bringing a specialized perspective in a highly complex center like Fundació Puigvert.

Diagnosis

Nursing and
patient experience

The nursing team systematically listens to and collects the voice of the patients and their families and caregivers through experience measures and patient-reported outcomes (PREMs and PROMs). This information, focused on the real experience of the different processes, is used to drive continuous improvement and to involve patients in defining care priorities.

Based on this feedback, Fundació Puigvert integrates patients into decision-making related to the design and adequacy of infrastructures, the definition of pathways, the review of protocols, and the organization of care, with the aim of providing care that is closer, safer, and aligned with what is truly important to them.

Programs for patients with Chronic Kidney Disease

Programs for urological patients and ostomy care

Programs for Sexual Health

Health education for people in the assisted reproduction program

The goal is for the person to feel accompanied, informed, and empowered before, during, and after their stay at Fundació Puigvert.
